Acceptable LabCorp Order Forms

MSL has worked with LabCorp for many years as a contract draw site, and we’re more than happy to try and help patients who have LabCorp orders. Unfortunately, we cannot draw or collect for patients that only have an accession number for their order, as seen to the left here. If all you have is an accession number, you will have to visit an actual LabCorp location, such as the ones in Marion and Carmel.

If your LabCorp order looks like the requisition seen to the left here, then we can accept this form.
